Health & Safety Requirements
Upon admission for grooming, pets must be free from any condition which could potentially jeopardize the health, safety, or well-being of other animals, our clients, and staff. By utilizing our services, the owner(s) represent, warrant, and certify that the pet being put into our care is in good health and has not been ill with a communicable condition in the last thirty (30) days. If you are aware that your dog is not feeling well, please refrain from bringing them to their grooming appointment until they are feeling better.
Grooming can be stressful to some pets. The owner must inform the groomer if their pet has any heart conditions or any stress-related issues before grooming.
Vaccination Requirements: For the safety of your pet and all of our other guests and staff, your pet MUST have the following vaccination:
• Rabies
Owners shall be required to submit proof of vaccination. We strongly recommend the use of heartworm preventatives and the Bordetella vaccine.
To protect the health and safety of your pet, all of our other guests and staff, your pet SHOULD:
• Be in good health.
• Be on a flea, tick, and heartworm prevention medication program
Dismissals
Your pet may be dismissed from a grooming appointment in the sole discretion of the staff for various reasons including, without limitation, aggression, excessive barking, illness, or other unsafe/unsanitary circumstances.
Permissions and Liability Waiver
I give permission for The Dogscape LLC to post videos and pictures of my pet on social media and its website.
Emergencies: In the event of an emergency, I authorize The Dogscape LLC to immediately seek professional veterinary attention for my pet (at my expense). I understand that all attempts will be made to contact me in the event of an emergency. The Owner(s) shall be responsible for, and shall pay in full, all expenses for any veterinary treatment incurred.
Coat Condition: I understand that The Dogscape LLC puts my pet's comfort and safety above all else. If my pet's coat is severely matted, I understand that the groomer may have to shave the mats out rather than perform a painful de-matting procedure. I also understand that if my pet is severely matted, there is an increased risk of skin irritation. I understand that all attempts will be made to prevent this; however, in many extreme conditions, it is unavoidable. I understand that matted pets take additional time to groom, which could result in an additional fee added to the regular grooming price if my pet's coat is matted.
General Liability: The Owner(s) understands, acknowledges, and agrees that grooming services can carry inherent risks and that a pet may be injured. In consideration of, and as material inducement for, the provision of grooming services, the terms and conditions hereof, and our obligations hereunder, the Owner(s), for himself/herself together with all heirs, successors and assigns hereby expressly and irrevocably releases, remises, and acquits The Dogscape LLC, its members, managers, agents, employees, consultants, independent contractors, attorneys, advisors, successors, and assigns, jointly and severally, from any claims, demands, causes of action, obligations, damages or liabilities arising from or in connection with your pet’s grooming appointment or our provision of services under this agreement.
Cancellation Policy: I understand that if I need to change my appointment time or cancel it, I must give at least 24 hours’ notice, allowing The Dogscape LLC to fill that appointment time and to make it available to another client. If two appointments are missed without giving notice, clients may then be required to pre-pay prior to scheduling future appointments.
I hereby certify that I am the actual owner of the pet for which this agreement (together with the waivers and releases contained herein) applies. I certify that I have carefully read and understand all of the statements contained in this agreement for the provision of animal grooming services.
